Instructions

An HTML editor that can display public-facing instructions, images, or other content

An instructional block may inform the submitter about how to complete the following form section.

Text Box

A form field that allows free text to be entered. Only system fields set up as text fields will display for mapping.

A text box may collect the form submitter's school name.

Paragraph Text

A form field that allows free text to be entered. Its height and width can be adjusted, and only system fields set up as text fields will display for mapping.

A paragraph text box might collect additional information needed from a form submitter.

Check Boxes

A form field that displays prompt options as check boxes where multiple items can be selected. A maximum number of options that may be selected can be set, and only system fields set up as prompt-driven fields will display for mapping.

Check boxes could be used to collect multiple selections for athletic interests.

Selectable

A form field that displays prompt options as selectable bars where multiple items can be selected. A maximum number of options that may be selected can be set, and only system fields set up as prompt-driven fields will display for mapping. Changing the colors of the "Unselected" and "Selected" boxes allows form registrants to make selections.

A selectable form field could be used to conditionally display other form fields based on its selected prompts.

Option Buttons

A form field that displays prompt options as "buttons," where only one option can be selected. Only prompt-driven fields will display for mapping.

Option buttons could be used to collect the record's student type.

Rating Scale

A form field that displays prompts as option buttons horizontally, where only prompt-driven fields will display for mapping

A series of rating-scale form fields might be featured on a recommendation form to collect applicant ratings.

Select List

A form field that displays prompts in a drop-down list where only one option can be selected. Only prompt-driven fields will display for mapping.

A select list may be used to collect entry term.

Multi-select List

A form field that displays prompts as rows within a box where multiple items can be selected. Prompts appear in a scrollable list and may be grouped or limited by category. Only system fields set up as prompt-driven fields will display for mapping.

A multi-select list may be used to select extra-curricular interests.

Street Address

A form field that collects address data, with country appearing first, followed by street, city, state/region, and postal code.

In the settings for Street Address, an Allow Copy From option enables the user to copy address data from another Street Address field on the form.

Street address may collect the mailing address of the form submitter.

Location

A form field that collects country, city, state/region, and postal code, or just country, city, and state/region.

The location form field may collect the desired location of alumni interviews.

Birthdate

A form field that collects month, day, and year, with the year prompts ranging from 1900 to the current year.

A birthdate form field could collect the birthdate of a relationship entered on the form.

Date

A form field that collects date data, including the month, day, and year, where the year prompts can be adjusted. The form field type can be set to Date (Year-Month) or Date (Month-Day) for less granular date collection. By default, Form fields with a type of "Date" store data using a "yyyyMMdd" format, such as "20210402".

A date form field might collect the date when the record is graduating from their school.

Calendar

A form field that displays a calendar and allows a selection of a date, with its default set to the current month and day. By default, Form fields with a type of "Calendar" store data using a "yyyy-MM-dd" format, such as "2021-04-02".

A calendar form field could be used for an upcoming event date selection.

Section Break

An internal marker indicating a separate section on a form, and possibly including conditional logic.

A section break might be placed before a set of form fields that should display conditionally based on the response to a previous question.

Related Events Selector

A widget that displays related events in specified folders for which the form submitter may register.

A related events selector may display campus tours available during an open house event.

Material Uploader

A widget that allows the form submitter to upload material to the specified material type on their record.

 

📝 Note

The Material Upload Limit setting will only be honored if the form is scoped to Application Page or Slate-Hosted Application Page and is used within the Slate-Hosted Application.

A form submitter may submit an essay within an Application Creation form using a material uploader.

If a form submitter must upload multiple types of materials such as an essay and a resume, drag in one material uploader per material type.

Interaction Selector

A set of form fields that allows the form submitter to select an interaction code, set an interaction date, and leave comments related to the interaction to be posted to the record. This is meant to be used internally on a form, rather than shown externally to the public.

On a call form, the interaction selector may be featured for a user to leave notes on the call.

Widget Table

A widget to feature the selected widget-scoped form (such as a school widget, relationship widget, or entity widget). This form field type should only be used on Person Page- and Application Page-scoped forms.

A widget table may be placed on a form to feature a custom schools widget within a Slate-hosted application.

Translation

A tool that provides the translation from one data point to another. This form field type is used in conjunction with Translation Codes.

A translation may be placed on a form to translate a fee waiver code to an associated payment amount.

Payment

A widget that allows the form submitter to enter payment information, or that displays and stores a payment-due amount on the record.

A payment widget may be placed on a form to collect the required payment for an event associated with the form submission.

Video Capture

A widget that starts video recording on the submitter's device. A question for the form submitter can be included, a start timer can be set to count down before recording begins, and recording length can be capped.

A video capture widget can be used to capture video answers to interview questions from form submitters.

Secret

The masking character used to hide a value can be customized. The number of visible characters of the original string can also be customized, and will be visible to administrators.
